Our client has an exciting opportunity for a Support Worker to join their teambased in Nunhead, London. You will join them on a part time (30 hours per week), permanent basis in return you will receive a competitive salary of £20,573 per annum, pro rata.
The Support Worker role:
The scheme has individual rooms with shared facilities. There are two large kitchens with dining rooms, shared bathrooms, two lounges and a games room along with a garden and specialist support staff on site 24 hours a day. The support provided is driven by customers with a tri-part contract in place: the local authority, our client and the customer. Each customer creates a weekly schedule (also called an Individual Service Fund) based on what they want to achieve and who they want to support them with this. Customers also choose the time and day they wish their support to fall on. The support provided varies, from supporting you to go shopping and pay bills to going out to socialise. The ultimate goal of the service is to empower customers by learning new skills and achieving greater independence.
Responsibilities of our client's Support Worker include:
- Carry out needs and risk assessments, complete personalised (and where required pictorial) support plans, provide 1:1 support, and carry out reviews
- Hold a caseload of customers and provide creative and individualised support to them on a key-work basis in their own homes or at an alternative venue if required
- Provide practical and emotional support to customers to achieve the outcomes set out in their support plan
- Support customers with their personal care needs as per their support plans
- Support customers with their medication as per their support plans
- Providing specialist care as required including catheter and stoma care, wound and pressure area care, tracheotomy care, PEG feeding care and palliative and end of life care, if appropriate
- Physical assistance to assist customers with eating and drinking, going to the toilet, washing or bathing, dressing, oral care and the care of skin, hair and nails (not nail cutting) if required
- Cleaning and house care, including vacuuming, sweeping, washing up, polishing, cleaning floors and windows, bathrooms, kitchens, toilets etc and general tidying
- Support customers to identify, move into, and sustain housing which meets their Page 2 of 4 needs, including supporting customers to 'bid' for properties and report maintenance issues
- Support customers to maximise their income and to take control of their own finances; to access benefits, and to budget to cover domestic bills and living expenses
What they're looking for in their Support Worker:
- Experience of providing support to the vulnerable adults either in a volunteering or professional role is desirable
- Ability to communicate effectively and positively with customers, staff and stakeholders in plain, easy to understand English, both in writing and verbally. This may include the use of pictorial resources to communicate with customers
- Ability to deliver a range of practical skills based exercises to support customers to learn independent living skills- money management, keeping a home, travel training
- Ability to positively manage behaviours that may challenge you and to be able to positively diffuse conflict
- Ability to work flexibly to meet customer needs and service requirements, including working evenings and weekends where the service requires it
- Demonstrable commitment to equality, diversity and inclusion and a genuine desire to help people with disabilities and/or support needs connect with their community
- Comprehensive IT skills, including the use of Microsoft Office and the ability to maintain
comprehensive electronic records.
