One year fixed term contract / secondment
This role is based within Adult Services within the East London Foundation Trust. Your role will be required to assess the needs of carers in accordance with the wellbeing principles of the Care Act and offer support using a personalised and strengths-based approach. This role will also involve the development of a detailed carers provision within ELFT that is compliant with statutory requirements and relevant legislation.
The Carers Lead will work to promote personalised support whilst managing a case load and conducting robust assessments in partnership with carers and multi-disciplinary partners. The Carers Lead will also clearly identify outcomes that need to be achieved through provision of this service. They will also help people identify how best to meet their own longer-term needs and aspirations by enabling them to identify appropriate asset-based goals and building outcomes focused support plans.
The Carers Lead will support team members in maintaining and developing high professional standards whilst working with carers, and have the ability to provide effective mentoring, shadowing, teaching, learning and leadership opportunities to others.
A detailed understanding of the Care Act is also required, combined with the ability to work in an integrated multi agency manner. The Carers Lead will also have experience delivering Adult Social Care including adult safeguarding, BIA/MCA work or Mental Health/AMPH.
